Head of Case Management, Case Manager & OT
Tracy is a qualified Occupational Therapist with over 25 years of experience working within the health and social care sector (statutory, private, and independent). Working within the fields of adult and elderly mental health, learning disabilities, orthopaedics, and stroke rehabilitation, supporting all related associated factors of vulnerability, mobility, and communication difficulties.
Within the NHS, Tracy worked up to the level of management and development of an Occupational Therapy adult mental health service. Within this role, Tracy managed a complex caseload spread across acute inpatient to community-based services. Supervising staff and students, working closely with multi-disciplinary and multi-agency teams.
In 2015, Tracy started working alongside Stokes Case Management as an associate, utilising her occupational therapy skills to offer case management and occupational therapy services to individuals with a range of severe and complex needs. As an occupational therapist, she uses her knowledge of how illness and disability affect daily structure and routine and uses her excellent communication skills to encourage individuals to engage in their rehabilitation. In 2021, Tracy stepped into the role of Head of Case Management with Stokes Case Management. Within this role, she utilises her clinical reasoning and mentoring skills to guide and support all the Case Managers that work with Stokes Case Management. Alongside the role of Head of Case management, Tracy continues to hold a small caseload of complex care clients and therefore maintains her skills in coordinating and supporting clinical teams of clients with complex needs, in both settled and unsettled cases, of adults and children. Working closely with families/carers and other agencies to educate on and coordinate care packages/ support services as required.
